Statement from Nominee as to why he/she is interested in membership in the Servetus International Society, and what the nominee would hope to contribute to the Society.

A short curriculum vitae that includes a list significant publications which pertain to 1) Servetus, 2) other publications.

Copy of the candidate's most significant publication pertaining to Servetus and/or other topics considered of relevance.

Payment of the regular membership fee of €25 a year
